About the role

We’re seeking a Medication Aide/Care Partner to provide medication management and support Activities of Daily Living (ADLs) for residents in Assisted Living and Memory Care. In this role, you’ll deliver care in accordance with resident service plans, communicate with medical professionals and families, and ensure residents’ safety and well-being while supporting regulatory compliance.

Responsibilities

  • Follow each resident’s service plan in a friendly and supportive manner, including passing medications per physician orders and Medication Administration Records
  • Document completion of daily tasks and observed changes in residents’ conditions
  • Communicate resident needs with other Medication Aides to ensure consistent care
  • Participate in meal service, including serving, clearing, and cleaning the dining room
  • Manage staff under your supervision during working hours
  • Maintain required certifications and attend ongoing training and in-service programs
  • Comply with the Continuous Quality Improvement program and report issues to your supervisor
  • Follow all local, state, and federal rules and regulations, including HIPAA, OSHA, and Blood Borne Pathogen Guidelines
  • Report suspected elder maltreatment or abuse to the Director of Health and Wellness, Executive Director, and/or State
  • Work safely and foster a safe work environment for all employees
  • Support teamwork, take initiative, and maintain a positive impact on residents, visitors, and coworkers
  • Perform other duties as assigned

Requirements

  • A High School Diploma or GED
  • Minimum one year of experience in a caregiving role or similar function
  • At least six months of supervisory experience in a health-related field
  • Ability to read physician orders and match them to Medication Administration Records
  • Understanding of medical terminology adequate to administer medications and communicate with medical professionals
  • Certifications as required by state
  • Must be 18 years or older and meet all health requirements, including TB clearance
  • First Aid certification required

Compensation is commensurate with experience.
