Medication Access Coordinator

ESSENTIAL FUNCTIONS

  • Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.
  • Deliver patient care through patient preparation, vital sign collection and documentation.
  • Absorb and assist with various procedures, exams, and diagnostic equipment (e.g., tympanometry, spirometry).
  • Maintain aseptic techniques during preparation, procedures, and medication administration.
  • Review payer reports and determine appropriate course of action for management.
  • Determine daily task priorities based on business and patient/customer needs.
  • Triage patients' needs and assign to appropriate staff.
  • Communicate with external entities as needed.
  • Perform all insurance benefit investigations, review claim rejects, overrides.
  • Work with patients to find financial aid (grants, manufacturing programs, discount cards etc.) to ensure they have access to the medications they need.
  • Provide direct support to patients to ensure that they are supported through their therapy regimen and achieve their intended outcome.
  • Serve as a liaison between clinicians, care coordinators, pharmacy, and assistance programs to ensure patient support throughout the duration of their care.
  • Create and/or maintain detailed resources and documents related to business operations and workflow.
  • Access, input, and retrieve information through the Electronic Medical Record and Care Management computer network to maintain accurate records.
  • Participate in scheduled department meetings.
  • Conduct outreach calls to patient and pharmacy staff.
  • Other duties as needed and assigned by the manager.

MINIMUM QUALIFICATIONS

  • Must be comfortable operating in a collaborative, shared leadership environment.
  • High School diploma or GED required.
  • Must be at least 18 years of age.
  • Must be able to pass a pre-employment drug screen at time of hire.
  • The Iowa Board of Pharmacy Certified Pharmacy Technician License required.
  • 2-3 years of pharmacy experience. (Community, Inpatient, Ambulatory, Insurance. Etc.)
  • Knowledge of health benefit investigations, prior authorization support, patient financial assistance, medical terminology, diagnoses, and procedures.
  • Proficiency in Microsoft Office products.
  • Knowledge of Electronic Medical Records, health care payers, and specialty pharmacy operations is highly preferred.
  • Completion of Mandatory Reporter training for child and dependent adult abuse within three (3) months of hire.
  • Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health.
