Medication Access & Care Coordinator

As a Medication Access & Care Coordinator, you'll be part of the Medication Care Program Team that ensures patients with complex medication needs receive timely, affordable, and well-coordinated pharmacy care.

Responsibilities

• Coordinate care across patients, providers, pharmacies, and payors using technology, communication skills, and clinical insight to keep therapies aligned and avoid unnecessary disruption.
• Identify opportunities to simplify medication schedules.
• Engage patients in ongoing support.
• Ensure financial and billing pathways are clear and accurate.

Requirements

• Passionate about helping patients get the medications they need, when they need them.
• Experience in a similar role or relevant field preferred.
• Strong communication and organizational skills.
• Attention to detail and ability to multitask.

Qualifications

• Bachelor’s degree in Pharmacy, Healthcare Administration, or related field.
• Current license to practice pharmacy in Indiana.

Skills

• Excellent written and verbal communication skills.
• Ability to work independently and as part of a team.
• Proficiency in using pharmacy software and electronic health records.

Pay

$15.00 - $17.00 per hour, depending on experience.

Schedule

Monday through Friday, approximately 8:00 a.m. to 4:30 p.m., with no weekends or holidays.
