Medicare Home Health Registered Nurse for Brevard County

About the Role

Activa Home Health (an AccordCare Company), a licensed Medicare certified home health agency, is seeking experienced field Registered Nurses (RNs) for part-time work in the Sebastian and Barefoot Bay area. WellSky/Kinnser, Oasis, and Home Health experience is a plus.

Responsibilities

• Provide safe and effective care for the client based on the individualized Plan of Care.
• Communicate promptly any changes in patient medical status or emergencies to the Team for effective case management.
• Deliver safe and effective nursing care within the scope of practice and individualized Plan of Care, in conjunction with the Nurse Supervisor/Manager of Clinical Practice. Duties may include, but are not limited to:
  • Medication administration
  • Wound care
  • Infusion therapy
  • Transferring and positioning patients
  • Catheter care
  • Effective communication with the client, family, and Team Case Manager
• Demonstrate empathy, caring, and self-confidence; pay attention to details for thorough follow-through.
• Anticipate problems, know when to diffuse or escalate, prioritize tasks, and demonstrate a sense of urgency.

Requirements

• Current RN license in the state of Florida.
• Wound care and infusion skills.
• Excellent communication, interpersonal, and collaboration skills.
• Strong customer service skills.
• Proficiency in using electronic health records for documentation and communication regarding client care.
• 1 year of Home Health experience preferred.
