Medical Social Worker - Social Services - Day Shift Including Every 4th Weekend Both Saturday and Sunday

Duties

  • Identifies patients with barriers to a safe discharge plan.
  • Completes psychosocial assessments on all patients.
  • Documents all clinical assessments, notes, and referrals in Epic.
  • Completes screens and coordinates all new skilled nursing facility and rehab facility referrals.
  • Assesses all cases involving child abuse/neglect, domestic violence, elderly abuse, and sexual assault.
  • Educates patient/family about post-acute care options and the role of choice.
  • Educates patient/family about managed care and payor issues

Preferred Criteria

  • At least one year of experience in an acute care setting
  • Discharge planning experience
  • Experience with Epic and Allscripts

Requirements

  • Master’s Degree in Social Work (MSW)
  • New York State License as a Licensed Masters Social Worker (LMSW).
  • Communication and negotiation skills
  • Ability to work independently and collaboratively with the interdisciplinary team
  • Work efficiently and effectively under pressure
  • Work comfortably within the limitations of the medical setting
  • Computer literacy

Pay

This is a full-time day shift position 9:00am-5:00pm Monday-Friday including every 4th weekend, both Saturday and Sunday, located at NewYork-Presbyterian Brooklyn Methodist Hospital. Salary Range $44.16-$53.81/Hourly
