About the role
The Medical Social Worker provides psychosocial assessment, case management, and resource coordination services to address barriers to patient’s optimal health related to Social Influencers of Health (SIOH). Connects patients with designated community agencies to meet social and financial needs, and provides advocacy, crisis intervention, and resource assistance to patients and families. This position will be fully in person at the West Arbor Pediatrics clinic.
Responsibilities
- Assess patients using screening tools and clinical interview regarding psycho‑social factors.
- Provide advanced problem solving and refer patients to community agencies to address needs and improve outcomes.
- Intervene with patients and their family/support system to alleviate barriers to health.
- Broker with community agencies to access services to meet patient’s needs.
- Coordinate care across interdisciplinary team of providers.
- Assess and triage patient needs by phone and in‑office visits.
- Connect patients with designated community agencies to meet social and financial needs.
- Provide advocacy, crisis intervention, and resource assistance to patients and families.
Requirements
- Master’s degree in social work from a school accredited by the Council of Social Work Education (CSWE).
- Valid, unrestricted license to practice clinical social work in the State of Michigan.
- Minimum 2 years of experience in a social work setting with vulnerable populations.
- Preferred experience in crisis intervention and medical settings, preferably with pediatric populations.
