Medical Social Worker , MSW Hospice - Masters Degree

Responsibilities

  • Aids the patient and family with health-related financial, social, and emotional concerns.
  • Provides social work services in alignment with the patient’s care plan.
  • Ensures timely communication of changes in the patient’s clinical status to other healthcare team members and care center office staff.
  • Educates the patient and family members, as well as other team members, on medical social work interventions and treatment plans.
  • Performs care planning, transfers, and discharges as needed.
  • Identifies potential risks or threats to the patient’s health and implements appropriate interventions to mitigate these risks.
  • Communicates identified risks/threats to the physician and other members of the healthcare team to facilitate effective care coordination.

Qualifications

  • Holds a master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education.
  • Has one year of social work experience in a healthcare setting.
  • Maintains compliance with state and federal regulations for this position.
  • Carries current CPR certification.
