Responsibilities
- Aids the patient and family with health-related financial, social, and emotional concerns.
- Provides social work services in alignment with the patient’s care plan.
- Ensures timely communication of changes in the patient’s clinical status to other healthcare team members and care center office staff.
- Educates the patient and family members, as well as other team members, on medical social work interventions and treatment plans.
- Performs care planning, transfers, and discharges as needed.
- Identifies potential risks or threats to the patient’s health and implements appropriate interventions to mitigate these risks.
- Communicates identified risks/threats to the physician and other members of the healthcare team to facilitate effective care coordination.
Qualifications
- Holds a master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Has one year of social work experience in a healthcare setting.
- Maintains compliance with state and federal regulations for this position.
- Carries current CPR certification.
