Responsibilities

• Assess the psychosocial status of patients related to the patient's illness and environment.
• Carry out social evaluations and plan intervention based on evaluation findings.
• Maintain clinical records on all patients referred to social work.
• Provide information and referral services for the organizations' patients and families/caregivers regarding practical and environmental needs.
• Provide information to patients or families/caregivers and community agencies.
• Serve as a liaison between patients or families/caregivers and community agencies.
• Maintain collaborative relationships with the organization's personnel to support patient care.
• Participate in the development of the total plan of care and case conferences as required.
• Assist physician and other team members in understanding significant social and emotional factors related to health problems.
• Participate in discharge planning.
• Other duties as delegated by the Director of Hospice/Director of Patient Services.

Requirements

• Graduate of a master's program in social work accredited by the Council on Social Work Education (CSWE).
• Minimum of one year's experience in health care, preferably Home Health Care.
• Experience in home health highly preferred.
• Demonstrates good verbal and written communication and organization skills.

The coverage area includes the Peninsula and the Southside.
