Responsibilities
- Assess the psychosocial status of patients related to the patient's illness and environment.
- Carry out social evaluations and plan intervention based on evaluation findings.
- Maintain clinical records on all patients referred to social work.
- Provide information and referral services for the organizations' patients and families/caregivers regarding practical and environmental needs.
- Provide information to patients or families/caregivers and community agencies.
- Serve as a liaison between patients or families/caregivers and community agencies.
- Maintain collaborative relationships with the organization's personnel to support patient care.
- Participate in the development of the total plan of care and case conferences as required.
- Assist physician and other team members in understanding significant social and emotional factors related to health problems.
- Participate in discharge planning.
- Other duties as delegated by the Director of Hospice/Director of Patient Services.
Requirements
- Graduate of a master's program in social work accredited by the Council on Social Work Education (CSWE).
- Minimum of one year's experience in health care, preferably Home Health Care.
- Experience in home health highly preferred.
- Demonstrates good verbal and written communication and organization skills.
The coverage area includes the Peninsula and the Southside.
