Medical Social Worker-MSW

About the role

The Social Work practice that occurs in hospitals and other health care settings inspires health, and aids physically ill patients and their families. The Social Worker assists in resolving the social and psychological problems related to the illness. The Social worker sensitizes other health care providers about the social and psychological aspects of illness. The Social Worker empowers patients to be proactive with regard to their health and personal well-being.

Responsibilities

• Provides clinical coordination and therapeutic interventions through assessment, referral and interdisciplinary planning for high-risk populations.
• Links patients and families to systems that provide resources, services and opportunities to achieve and maintain maximum functioning.
• Expedites linking patients with systems in the community to provide them with resources, services and opportunities to achieve and maintain maximum functioning.
• Conducts screenings for CBT resource linkage.
• Provides counseling and support to patients and families regarding adjustments to disease process and illnesses.
• Provides field instruction for MSW students.

Requirements

Education:

• Masters Degree in Social Work required.

Experience:

• Minimum of 1-year clinical experience in medical or healthcare setting.

• Ability to effectively multi-task and prioritize responsibilities.
