Medical Social Worker (MSW)

About the role

We are seeking a Medical Social Worker (MSW) to join our home health team. This role is available on a per diem basis, 1 day a week, with 5-6 patients per day, covering territories in Macedonia, Twinsburg, Parma, Cleveland and surrounding areas.

Responsibilities

• Complete comprehensive patient assessments.
• Participate in developing individualized Plans of Care.
• Identify barriers to care and recommend appropriate interventions.
• Connect patients and families with valuable community resources.
• Aid in financial assistance applications and long-term planning.
• Educate and support patients and caregivers throughout their care journey.
• Provide counseling related to adjustment, coping, loss, and grief.
• Collaborate with physicians, nurses, therapists, and the interdisciplinary care team.
• Accurately document clinical information according to agency, Medicare, and federal guidelines.
• Monitor and report changes in patient conditions.
• Attend required training and agency meetings.

Requirements

• Current Medical Social Worker (MSW) license in the state of employment.
• Master's Degree in Social Work (MSW) required.
• One year of MSW experience in an acute care setting preferred.
• Home health experience preferred.
• Valid driver's license and current auto insurance.
• Ability to meet the physical demands of the position.
