Medical Social Worker LMSW PRN

Responsibilities

• Aids patients and families with health-related financial, social, and emotional concerns.
• Provides social work services in accordance with the plan of care.
• Ensures timely communication of changes in the patient's clinical status to other healthcare team members and care center office staff.
• Educates patients and family members, as well as other team members, on medical social work interventions and treatment plans.
• Performs care planning, transfers, and discharges.
• Identifies and addresses risks or threats to the patient's health, implementing appropriate interventions.
• Communicates identified risks/threats to the physician and other members of the healthcare team to coordinate care effectively.

Qualifications

• Holds a master's or doctoral degree from a school of social work accredited by the Council on Social Work Education.
• Has one year of social work experience in a healthcare setting.
• Mets state and federal requirements for this position.
• Carries a current CPR certification.

Pay

Compensation varies based on home health experience, ranging from $30 to $37.50 per hour.
