Responsibilities

• Function within the Interdisciplinary Treatment Team to lead and provide patient care and supportive services aimed at achieving patient care outcomes, maximizing patient involvement in their care plan, improving service levels, and decreasing costs.
• Provide clinical work with patients with mental health diagnoses, including individual, group, and couple/family therapy to assist patients in determining treatment goals, returning to previous or improved functioning, forming aftercare plans, and advocating within the local community if needed.
• Provide effective treatment services to a caseload of clients with mental illness, including diagnostic assessment and diagnosis within the guidelines of the DSM-5-TR.
• Spend at least 50% of time providing direct service to an established clinical caseload.
• Provide psycho-education groups related to general mental health as indicated.
• Assist clients in writing and meeting treatment plan goals and objectives.
• Work with clients to solve problems consistent with the philosophy of the treatment team and specific treatment goals.
• Provide individual, family, and group therapy services as indicated.
• Complete documentation consistent with standards set by the compliance office.
• Communicate observations and concerns about clients with team members as needed.
• Maintain positive contacts with referral sources and consult when needed.
• Request and provide consultation and/or support as needed from supervisors or team members.
• Attend and contribute to program staff and treatment team meetings.
• Attend staff in-services and other required meetings.
• Provide treatment reviews and updates to managed care payers.
• Document utilization reviews in the chart for clarity among team members and the Business Office.
• Provide clients with links to community resources to help meet their needs.
• Support community and healthcare linkage for each client (e.g., appointments with outpatient providers, psychiatrists, clinical nurse specialists, or primary care physicians).
• Represent the program or hospital by participating in committees, conferences, public meetings, and providing educational presentations to groups.
• Perform functions using job-related software and systems.
• Deliver evidence-based therapeutic interventions to patients experiencing substance use disorders, supporting them through various stages of recovery.
• Collaborate with medical providers to ensure holistic care for patients with addiction therapy needs.
• Apply familiarity with addiction services, treatment modalities, and the continuum of care (e.g., detox, inpatient, outpatient, recovery support).
• Utilize a strong understanding of the stages of change model in addiction therapy.

Requirements

• Master’s Degree in Social Work or Counseling.
• Independent Ohio license (LISW, LISW-S, or LPCC) to practice.
• At least two years of experience working with patients with mental illness.
• Expertise/training in cognitive-behavioral and/or interpersonal psychotherapy.
• Understanding of the psychosocial aspects of chronic illness.
• Familiarity and competency with assessment and diagnosis of mental health disorders.
• Strong organizational and documentation skills.
• Ability to work independently while also being a strong team player.

Pay

USD $66,002.00 - USD $99,004.00 per year.
