Medical Social Worker (LICSW)

Responsibilities

• Conduct psychosocial assessments and provide brief counseling and crisis intervention to patients and families.
• Develop and implement discharge plans to ensure smooth transitions of care.
• Collaborate with physicians and healthcare teams to coordinate patient support services.
• Connect patients to community resources and advocate for equitable access to care.
• Support diverse patient populations within Healthcare & Medical Services, Community & Social Services departments.

Qualifications & Skills

• Current LICSW license and relevant social work experience.
• Strong interpersonal and communication skills for compassionate patient support.
• Ability to work collaboratively within multidisciplinary teams.
• Commitment to continuous improvement and patient-centered care.
