About the role
Medical Social Worker III (LCSW) responsible for delivering advanced clinical social work services in an on‑site healthcare setting.
Responsibilities
- Conduct comprehensive psychosocial assessments.
- Provide counseling, crisis intervention, and short‑term therapy.
- Develop safe and effective discharge and care plans.
- Collaborate with physicians, nurses, and care teams to coordinate patient care.
- Refer patients to community resources and advocate for patient access and rights.
- Document all care activities accurately in the Electronic Health Record (EHR).
- Support quality, continuity of care, and optimal patient outcomes.
Qualifications
- Licensed Clinical Social Worker (LCSW) with relevant experience in healthcare.
- Strong assessment, counseling, and crisis intervention skills.
- Ability to collaborate effectively with interdisciplinary teams.
- Excellent documentation and advocacy skills.
