About the role
BAYADA Home Health Care is seeking a Licensed Medical Social Worker (MSW) for a per diem opportunity performing home health visits in our Nashville Visits Office. This position serves clients on a per-visit basis in Davidson and Williamson Counties, requiring approximately 10 visits per week during the hours of 8:30 AM–5:00 PM, Monday–Friday. Prior home care experience is preferred but not required.
As a Medical Social Worker, you will join a multi-disciplinary healthcare team providing skilled nursing and rehabilitative care to clients, enabling them to receive necessary medical care at home.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Assist the team in understanding the social and emotional factors related to clients’ health problems.
- Provide clinical social evaluations and plan appropriate interventions based on findings.
- Identify and refer clients and families to community resources to address practical and environmental needs.
- Act as an advocate for clients and families to navigate community systems.
- Participate in developing the total plan of care with the multidisciplinary team.
- Develop and maintain contracts with public and private agencies as resources for patients.
- Educate patients, families, or representatives on community resources to promote health, safety, well-being, and independent living.
- Accurately document observations, interventions, and evaluations using a state-of-the-art touchpad tablet.
Requirements
- Must be licensed by the TN Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.
- Minimum of a Master’s in Social Work with at least 2 years of community-based experience.
- Accreditation by the Council on Social Work Education (demonstrated by school transcript or degree).
- Home care experience is a plus.
- Demonstrated ability to read, write, and communicate effectively in English.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to assigned cases.
