Medical Social Worker -Home Health, Full time
About the role
As a Medical Social Worker at Dartmouth Health Home Care, you will become an integral part of a multi-disciplinary healthcare team providing skilled nursing and rehabilitative care to patients in the comfort of their own homes. You’ll have the opportunity to build meaningful relationships with patients, offering 45-minute visits that allow for impactful therapy and support.

Responsibilities
- Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.
- Identify and refer patients and families to community resources to address practical and environmental needs.
- Act as an advocate for patients and families, helping them navigate community systems.
- Participate in developing the total plan of care with the multidisciplinary team of clinicians.
- Develop and maintain contracts with public and private agencies as resources for patients.
Qualifications
- Master of Social Work (MSW) required.
- Minimum one year of experience in a health-related field.
- Experience with database software or electronic medical record (EMR) software.
- Valid driver’s license, minimum car insurance requirements, and a clean driving record.
Preferred Qualifications
- Licensed Clinical Social Worker (LCSW) preferred.
- Hospice experience preferred.
