Medical Social Worker -Home Health, Full time

About the role

You really get to know your patients and become an integral part of their care team. Your average visit is 45 minutes, giving you significant time to provide meaningful therapy. You are provided broad independence but are also part of a cohesive patient-care team. You get to be an important part of the community where you live. Responsibilities

• Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.
• Identify appropriate community resource referrals to address the client’s and the family’s practical and environmental needs.
• Act as an advocate for both the client and the family, to navigate the patient through the community system.
• Participate in the development of the total plan of care with the multidisciplinary team of DHHC clinicians, as appropriate.
• Develop and maintain contracts with public and private agencies as resources for the patient.

Requirements

• Master of Social Work (MSW) required
• Minimum one year experience in a health-related field required
• Experience with database software or electronic medical record software required

Preferred Qualifications

• LCSW preferred
• Hospice experience preferred

Licenses

• Driver’s license, minimum requirements on car insurance, and a clean driving record
