About the role
The Medical Social Worker at Trinity Health provides consultation and service to homebound individuals, assisting with the solution of complex social and emotional problems that impede medical regimen management.
Essential Functions
- Assesses social history and identifies patient needs through interviews, medical record review, and consultations with physicians and staff.
- Reflects advanced competency in psychosocial assessment and diagnosis, analyzing patient situations appropriately.
- Develops individualized psychosocial case plans in collaboration with the health care delivery team and implements and monitors progress.
- Coordinates discharge planning services, including referrals to community resources, and supports patients and families during the transition process.
- Demonstrates ability to respond to crises and emergencies, providing immediate supportive counseling and assistance.
- Provides psychosocial counseling and treatment services to promote optimal social and physical health adjustment.
Minimum Requirements
- Master's degree in social work or related field required.
- Two years experience in medical social work preferred.
- Competence in advanced medical social work services, including psychosocial counseling with individuals and families.
- Skills in conducting clinical interviews and understanding medical terminology.
- Knowledge of social work principles, standards, and ethics.
- Basic understanding of Medicare and Medicaid regulations and child/dependent adult laws and regulations.
- Valid Ohio driver's license and automobile liability insurance required.
