How You’ll Make an Impact
Aid the team in understanding the social and emotional aspects of clients' health issues.
Conduct clinical social evaluations and develop appropriate intervention plans based on the evaluations.
Identify community resources to assist clients and their families with practical and environmental needs.
Advocate for clients and their families to navigate community systems and ensure they have access to necessary resources.
Collaborate with the multidisciplinary team to develop comprehensive care plans for clients.
Provide education and guidance to patients, family members, or other patient representatives regarding available community resources.
Accurately document observations, interventions, and evaluations of patient care using a touchpad tablet.

Qualifications
- A minimum of a master's degree in social work, with at least two (2) years of community-based experience.
- Accreditation by the Council on Social Work Education, as demonstrated by school transcript or degree.
- Home Health Care experience a plus.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases, as assigned.
