Medical Social Worker (BSW or MSW); Full-time, MMMC

Job Summary

About the role

Serves as a social worker providing services to patients and their families in a healthcare setting.

Responsibilities

• Conduct psychosocial assessments to identify patient and family needs.
• Develop and implement social work service plans.
• Carefully coordinate discharge planning and care transitions.
• Manage complex cases involving various barriers to discharge.
• Collaborate with community agencies and legal resources.
• Provide crisis intervention and emotional support.
• Facilitate family meetings and provide case management.
• Advocate for patient rights and assist with advance directives.
• Ensure compliance with regulatory standards and maintain confidentiality.
• Participate in interdisciplinary rounds and care conferences.
• Provide culturally competent care.
• Document according to departmental standards.
• Participate in performance improvement activities.
• Maintain educational records and demonstrate competency.
• Attend departmental meetings and assist with orientation.

Requirements

• Basic Qualifications: N/A
• Education: Bachelors Degree in Social Work and two (2) years of social work experience in healthcare setting OR Masters Degree in Social Work.
• Licenses, Certifications, Registrations: Licensed Bachelor Social Worker (Hawaii) OR Licensed Clinical Social Worker (Hawaii) OR Licensed Social Worker (Hawaii)
• Additional Requirements: Basic Life Support required at hire from American Heart Association

Qualifications

• Experience: Basic Qualifications: N/A
• Preferred Qualifications: Minimum one (1) year of social work experience. Certified Case Manager (CCM) or Accredited Case Manager (ACM) Certification.

Pay

Range: $39.39 - $49.25 / hour

Schedule

Full-time, 40 hours per week, Monday through Sunday, 8:00 AM to 4:30 PM.
