Job Summary
About the role
Serves as a social worker providing services to patients and their families in a healthcare setting.
Responsibilities
- Conduct psychosocial assessments to identify patient and family needs.
- Develop and implement social work service plans.
- Carefully coordinate discharge planning and care transitions.
- Manage complex cases involving various barriers to discharge.
- Collaborate with community agencies and legal resources.
- Provide crisis intervention and emotional support.
- Facilitate family meetings and provide case management.
- Advocate for patient rights and assist with advance directives.
- Ensure compliance with regulatory standards and maintain confidentiality.
- Participate in interdisciplinary rounds and care conferences.
- Provide culturally competent care.
- Document according to departmental standards.
- Participate in performance improvement activities.
- Maintain educational records and demonstrate competency.
- Attend departmental meetings and assist with orientation.
Requirements
- Basic Qualifications: N/A
- Education: Bachelors Degree in Social Work and two (2) years of social work experience in healthcare setting OR Masters Degree in Social Work.
- Licenses, Certifications, Registrations: Licensed Bachelor Social Worker (Hawaii) OR Licensed Clinical Social Worker (Hawaii) OR Licensed Social Worker (Hawaii)
- Additional Requirements: Basic Life Support required at hire from American Heart Association
Qualifications
- Experience: Basic Qualifications: N/A
- Preferred Qualifications: Minimum one (1) year of social work experience. Certified Case Manager (CCM) or Accredited Case Manager (ACM) Certification.

Range: $39.39 - $49.25 / hour
Schedule
Full-time, 40 hours per week, Monday through Sunday, 8:00 AM to 4:30 PM.
