Medical Social Worker - Bilingual Spanish Preferred

About the role
The Medical Social Worker is a key member of Oak Street Health Centers, contributing a unique and valuable perspective to care teams to support the psychosocial needs of patients. In collaboration with the interdisciplinary care team, they provide assessments, advocacy, and comprehensive care coordination services to highest-risk patients and their caregivers to improve health status and avoid hospitalizations.
Responsibilities
- Proactively support highest-need patients through comprehensive assessments, telephonic check-ins, and in-person visits.
- Partner with patients and their support persons to identify goals, increase engagement in healthcare, address barriers, and drive execution of care plans.
- Maintain extensive knowledge of community resources and make referrals to ensure patients' healthcare and psychosocial needs are met.
- Navigate patient insurance benefits with the Patient Relations Manager to ensure mindful utilization and maximization of plan benefits.
- Provide longitudinal and episodic comprehensive care coordination to empower patients to independently manage health conditions and improve quality of life.
- Educate patients on supportive care options, including advance care planning and end-of-life care, and provide goals-of-care counseling, support, and advocacy.
- Provide psychoeducation to patients and families on life-stage and disease-specific issues, including dementia, adjustment to illness, aging, and caregiver support.
- Collaborate with care teams to generate creative solutions to patient issues and spearhead interventions in complex psychosocial situations.
- Complete concise and comprehensive documentation on time to ensure visibility of MSW interventions for all care team members.
- Partner with Community Health Workers, providing clinical guidance and consultation on complex patient needs.
- Respond to crises and urgent needs with the ability to prioritize work, triage situations, and multitask.
- Other duties as assigned.
Requirements
- Master of Social Work degree from a school accredited by the Council on Social Work Education (CSWE).
- Minimally licensed in the state where the Oak Street Health Center is located.
- Fluency in Spanish.
- Proficient computer skills (including Microsoft, GSuite, and electronic medical records).
- US work authorization.
Preferred Qualifications
- Prior experience with complex care coordination and/or population healthcare management.
- Familiarity with using metrics and data to guide practice and drive outcomes.
- Minimum one year of medical social work experience in a healthcare environment, preferably within the last two years.
- Experience working on a multidisciplinary team of healthcare providers.

Pay
The typical pay range for this role is $51,256.00 – $111,351.00 annually. This range represents the base full-time salary and may include bonus, commission, or short-term incentive programs. The actual base salary offer will depend on factors such as experience, education, geography, and other relevant considerations.
