Essential Job Functions
- Affords psychosocial assessments and clinical interviews to identify psycho-social factors and mental health needs.
- Facilitates referrals to community agencies and resources to address patients' social, financial, and environmental needs.
- Develops, implements, and assesses continuing care plans to support patient health and wellness, collaborating with psychiatrists and therapists.
- Engages with patients and their families to resolve barriers to health and advocate for ethical and legal issues.
- Brokerage with community agencies to secure services for patients' psychosocial needs, coordinating care across interdisciplinary teams.
- Provides check-ins, crisis interventions, safety planning, risk assessments, and empowers patients to manage personal and social issues.
- Shares education and fosters understanding among mental health providers and staff about social influencers of health.
- Maintains up-to-date knowledge of legal issues impacting patients, such as divorce and custody laws, child/adult abuse reporting, and duty to warn.
Essential Qualifications
- Master’s degree in social work from an accredited school.
- Valid, unrestricted license to practice clinical social work in the State of Michigan.
- Minimum of 1 year experience in social work settings with vulnerable populations, preferred experience in crisis intervention and medical settings.
