Monday–Friday, 8:30 am–5:00 pm
Responsibilities
- Manage a caseload comprised of medically and culturally diverse inpatient and outpatient families.
- Assist families and patients by completing biopsychosocial assessments, identifying appropriate resources and referrals, assisting with discharge and transition planning, and serving as a liaison and advocate between the patient and various systems.
- Demonstrate knowledge of child development related to diagnosis, assessment, intervention, and referral of infants, toddlers, pre-school and school-age children, and adolescents to appropriate community agencies.
- Meet the needs of families as identified, presented, or requested by the interdisciplinary team or physician.
- Document information in the medical record consistent with departmental standards.
- Maintain a working knowledge of a variety of referral sources and community resources appropriate for the patient population within the state of Texas.
Requirements
- Bachelor’s degree or Master’s degree in social work from an accredited (CSWE) Social Work Program.
- Texas State Board of Social Work Examiners (TSBSWE) licensure: LBSW, LBSW-IPR, or LMSW.
- Minimum 1 year of experience in health care social work preferred.
