About the role

Responsibilities
- Assess patient needs and develop care plans
- Coordinate with healthcare providers to ensure continuity of care

- Facilitate communication between patients, families, and healthcare teams
- Document all interactions and care activities accurately
Requirements
- Bachelor’s degree in social work or related field
- Current license to practice social work in state
- At least 2 years of relevant experience in home health care or related field
- Strong interpersonal and communication skills
- Proficiency in Microsoft Office Suite
