Medical Social Worker

About the role

The Medical Social Worker, known internally as a Patient Advocate, facilitates and ensures continuity in patient care, serving as a conduit between the health center services and the community. This role strategically engages employees and their dependents before, during, and after visits to provide personalized, informative, and compassionate care. The Patient Advocate reports to the Patient Advocacy National Manager and collaborates closely with the Client Relations team, Clinical Operations, Health Center Providers, and Client HR teams.

Responsibilities

• Coordination of Care
  • Coordinate care with external specialists, imaging centers, labs, medical equipment providers, counselors, and community resources under the direction of the Provider.
  • Ensure electronic medical records are exchanged between Proactive MD’s Provider and external specialists.
  • Handle prior authorizations required by insurance companies for tests or procedures and send prior notifications to insurance regarding procedures or referrals.
  • Refine and update the list of value-based providers suitable for referral in the community.
  • Assist patients in navigating the healthcare system.
  • Coordinate care for patients admitted to or discharged from the emergency room or hospital, ensuring Proactive MD’s Provider is included in the aftercare process.
  • Visit hospitalized patients to ensure care coordination is directed by the Provider.
• Patient Engagement
  • Engage with patients to encourage use of the Health Center (via high-risk or patient engagement dashboards).
  • Encourage and coordinate follow-up visits at the Health Center.
  • Develop and execute outreach campaigns and educational resources in coordination with the on-site Provider, Client Relations, and Client representatives.
  • Represent the Health Center at internal and external meetings and events.
  • Encourage completion of patient surveys at the end of each Health Center encounter.
  • Send patient communications (text/email/mail) regarding significant events (e.g., birthdays, get-well messages, condolences, congratulations).
• Patient Education
  • Provide information to patients about available community services that could enhance their care.
  • Coordinate patient care and treatment plans; provide educational materials/consultation and follow up on patient well-being.
  • Assist patients in understanding benefits available through their health insurance program (e.g., EAP).
• Health Center Synergy
  • Coordinate with Clinical Operations during high Health Center utilization to ensure patients are greeted, checked in, and roomed, and phones are answered.
  • Serve as an “embedded” case manager for optimal utilization of skills and abilities in collaboration with the Provider.
  • Encourage patient completion of tablet-based patient satisfaction surveys.
  • Participate in Health Center-wide meetings to ensure excellent cross-communication and collaboration.

Requirements

• 2+ years of experience working in a healthcare setting, Medical or Healthcare Social Work, or a Case Management role.
• Bachelor of Social Work Degree or a degree in a related field such as Sociology, Psychology, or Counseling.
• Knowledge of the healthcare industry, available patient resources, and medical case management.
• Knowledge of HIPAA confidentiality requirements.

Monday - Friday, 8:00 AM to 5:00 PM.
