Medical Social Worker

About the role

NHC HomeCare Greenwood is seeking a Medical Social Worker to join their team. This position supervises and provides evaluations of social and emotional needs related to medical conditions, offering counseling and other support.

Responsibilities

• Assesses social and emotional factors to minimize impact on health and daily living
• Helps patients and caregivers understand and follow medical recommendations
• Aids patients and caregivers with personal and environmental issues that may lead to illness or hinder medical care
• Identifies and connects patients with community resources to help them return to community life, including discharge planning
• Collaborates with healthcare providers and home care teams to address significant social and emotional aspects of health problems
• Contributes to the development of the patient's care plan

Requirements

• Master’s Degree in Social Work from an accredited school
• One year of social work experience in a healthcare setting
• State-specific requirements:
  • Licensed by the South Carolina Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level
