Medical Social Worker

Responsibilities

• Conducts initial psycho-social assessments, develops individualized care plans, and establishes follow-up treatment goals.
• Identifies and reevaluates social, emotional, financial, and environmental factors impacting patient care.
• Communicates with internal contacts, utilizes community resources, educates patients and families on resource utilization, and provides discharge instructions.

Qualifications

• Master's degree in social work from an accredited institution or a baccalaureate degree with at least one year of healthcare social work experience.
• Preferably one year in home health care.
• Strong interpersonal and communication skills required.

Skills

• Knowledge of psycho-social principles, family dynamics, and community organization.
• Ability to work independently with travel as needed.
