Medical Social Worker

About the role

Cabrillo Hospice is seeking a compassionate and dedicated Social Worker to join its interdisciplinary hospice team. The role focuses on providing patient‑centered, dignified end‑of‑life care with an emphasis on emotional support, family education, and holistic well‑being.

Responsibilities

• Conduct comprehensive psychosocial assessments to evaluate emotional, social, and environmental needs of patients and families, including caregiver stress, family dynamics, coping abilities, and safety concerns.
• Provide compassionate counseling to patients and families navigating serious illness, grief, and end‑of‑life transitions.
• Collaborate closely with nurses, physicians, chaplains, and other care team members to develop and support individualized plans of care.
• Serve as a liaison to community agencies and support services, ensuring patients and families have access to appropriate resources.
• Participate in hospice bereavement services, offering follow‑up and resources to families after a loss.
• Maintain accurate documentation and ensure services align with hospice regulations, policies, and standards of care.

Qualifications

• Master’s Degree in Social Work (MSW).
• Hospice or healthcare experience preferred; geriatrics or end‑of‑life care is a plus.
• Strong interpersonal and communication skills.
• Compassionate, patient‑centered approach.
• Ability to work independently while collaborating within a multidisciplinary team.
• Cultural sensitivity and respect for diverse backgrounds and belief systems.

Pay

$33.00 – $39.00 per hour
