Medical Social Worker

Responsibilities

• Conduct psychosocial assessments
• Address social determinants of health
• Coordinate community resources
• Provide brief counseling and crisis intervention
• Develop care plans
• Collaborate closely with clinicians to enhance patient outcomes
• Support vulnerable patients by addressing social needs and facilitating access to resources
• Contribute to holistic care planning within a supportive, inclusive setting

Qualifications & Skills

• Strong interpersonal and communication skills
• Ability to work in a team-based, patient-centered environment
• Experience in social work or related fields
• Knowledge of community resources and crisis management (preferred)
