Medical Social Worker

About the Role

Blue Ridge Care Services is seeking a compassionate, mission‑driven Medical Social Worker to join its hospice team. The role involves advocating for, counseling, and supporting patients and families as they navigate end‑of‑life challenges, while collaborating with an interdisciplinary team to deliver individualized, dignified care.

Responsibilities

• Provide psychosocial assessments and develop individualized care plans.
• Offer emotional support, counseling, and guidance to patients and families.
• Help caregivers navigate stress, grief, loss, and end‑of‑life transitions.
• Connect patients with community resources and support services.
• Collaborate closely with nurses, physicians, chaplains, and other care team members.
• Advocate for patient needs and coordinate services across care settings.
• Maintain accurate documentation and participate in care planning meetings.
• Travel throughout the service area to provide support where patients call home.

Requirements

• Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or a related field.
• Minimum one year of healthcare experience.
• Strong communication, counseling, and organizational skills.
• Valid driver’s license, reliable transportation, and current auto insurance.

Preferred Qualifications

• Master of Social Work (MSW).
• Experience in hospice, palliative care, home health, or healthcare social work.
• Knowledge of hospice philosophy, grief support, and end‑of‑life care.
