Medical Social Work Masters prepared hospice

Responsibilities

• Aids the patient and family with health-related financial, social, and emotional concerns.
• Provides social work services in accordance with the plan of care.
• Communicates changes in the patient's clinical status to other healthcare team members and care center office staff.
• Educates the patient and family members and other team members on medical social work interventions and treatment plans.
• Performs care planning, transfer, and discharge functions.
• Identifies risks or threats to the patient's health and implements appropriate interventions.
• Coordinates care with physicians and other members of the healthcare team to address identified risks/threats.

Qualifications

• Holds a master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education.
• Has one year of social work experience in a healthcare setting.
• Mets state and federal requirements for this position.
• Currently holds a valid CPR certification.

Pay

Compensation reflects the cost of labor across several U.S. geographic markets and may vary depending on location, job-related knowledge, skills, and experience.

Schedule

PRN as needed.
