Medical Scribe

About the role

As a Medical Scribe (internally called Clinic Informatics Specialist) at Oak Street Health, you’ll support primary-care providers by documenting patient encounters in real time so they can focus on delivering exceptional care. Scribes join providers in the exam room, document structured notes (history, assessment, plan, exam), assign ICD-10 and CPT codes, and help close care gaps using population-health tools. The role is ideal for pre-med students or anyone pursuing careers in health informatics, public health, medical coding, or healthcare administration. Extensive on-the-job training is provided, and a 1–2 year commitment is expected.

Responsibilities

• Document patient encounters (~80%):
  • Observe and document visits in the exam room in real time.
  • Write structured notes covering history of present illness, assessment, plan, and physical exam.
  • Assign appropriate CPT and ICD-10 codes.
  • Prepare After Visit Summaries.
  • Consult with provider to ensure accurate, specific documentation.
• Improve clinical documentation (~10%):
  • Request and review medical records.
  • Use population-health tools to support documentation improvement.
  • Prepare for and support Daily Huddles and Clinical Documentation Reviews.
  • Consult with provider on documentation opportunities.
• Provide administrative support (~10%):
  • Place orders and referrals.
  • Address tasks for the provider and care team.
  • Assist with additional responsibilities related to clinical documentation.

Requirements

• Knowledge of medical terminology and common medications (required).
• Prior clinical experience—shadowing, volunteering, or scribe/transcription work (strongly preferred).
• Advanced listening and communication skills (required).
• Strong computer literacy and ability to learn new technical workflows (required).
• Fluency in Spanish, Polish, Russian, or other community languages (required where indicated).
• Ability to adapt to new workflows and learn quickly (required).
• Typing speed of 70+ words per minute (strongly preferred).
• Willingness to take direction and work as part of a patient-care team (required).
• Self-starter within role scope (required).
• Excellent in-person attendance; ability to work in clinics (required).
• Commitment to at least 1 year in the role (2+ years ideal) (required).
• Ability to work 40–45 hours per week during clinic hours (full-time) (required).
• Compliance with HIPAA and Oak Street Health policies (required).
• US work authorization (required).

Pay

The typical pay range for this role is $17.00 – $25.65 per hour, depending on experience, education, geography, and other factors. Eligible for CVS Health bonus, commission, or short-term incentive programs in addition to base pay.

Schedule

Full-time position with approximately 40–45 hours per week during clinic hours, including predictable hours and break times.
