Medical Scribe

About the role

As a Clinical Informatics Specialist (CIS or Medical Scribe) at Oak Street Health, you’ll support primary care providers with clinical documentation, allowing them to focus on delivering exceptional patient care. Scribes assist throughout the patient care journey—planning daily visits, documenting encounters in real time, and following up on next steps. Beyond traditional scribe duties, you’ll serve as a clinical documentation assistant, helping providers identify and close care gaps while becoming an expert in value-based care, chronic condition documentation, and ICD-10/CPT coding.

This role offers extensive on-the-job training in clinical workflows, value-based medicine, preventative care, and population health. It’s ideal for pre-med individuals seeking paid clinical experience before applying to MD/DO/PA/NP programs, as well as those pursuing careers in health informatics, public health, healthcare administration, or medical coding. A commitment of at least 1–2 years is preferred.

Responsibilities

• Documenting Patient Encounters (~80%)
  • Join providers in the exam room to observe and document patient visits in real time.
  • Record structured notes, including history of present illness, assessment, plan, and physical exam.
  • Assign appropriate CPT and ICD-10 codes.
  • Prepare After Visit Summaries.
  • Consult with providers to ensure accurate and specific documentation.
• Clinical Documentation Improvement (~10%)
  • Request and review medical records.
  • Use Oak Street’s population health tools to support documentation improvement.
  • Prepare for and participate in Daily Huddles and Clinical Documentation Reviews.
  • Consult with providers on documentation opportunities.
• Administrative Support (~10%)
  • Place orders and referrals.
  • Address tasks for the provider and care team.
  • Support additional responsibilities related to clinical documentation.
  • Perform other duties as assigned.

Requirements

• Knowledge
  • Medical terminology and common medications (required, from pre-medical degree or prior clinical experience).
  • Prior clinical experience, including shadowing or volunteering (strongly preferred).
  • Prior scribe or transcription experience (preferred but not required).
• Skills
  • Advanced listening and communication skills (required).
  • Strong computer literacy and ability to learn new technical workflows (required).
  • Fluency in Spanish, Polish, Russian, or other community languages (required where indicated).
• Abilities
  • Adaptability to new workflows and quick learning of new concepts (required).
  • Typing speed of 70+ words per minute (strongly preferred).
  • Willingness to take direction and collaborate as part of a patient care team (required).
  • Self-starter within role scope.
  • Excellent in-person attendance (required; providers rely on you).
  • Commitment to at least 1 year in the role (2+ years ideal).
  • Ability to work 40–45 hours per week during clinic hours (full-time, predictable schedule).
  • Compliance with HIPAA and Oak Street Health policies (required).
  • US work authorization (required).

Pay

The typical pay range for this role is $17.00–$31.30 per hour. The actual offer will depend on factors such as experience, education, and geography. This position is also eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay.

Schedule
