Job Function
- Reviews the release of requested medical records and ensures that the process is HIPAA compliant and timely
- Maintains and protects sensitive and private information by keeping patient records confidential in accordance with ambulatory care, The Joint Commission, HIPAA, local, state, and federal laws and regulations
- Investigates possible violations of HIPAA policies and procedures and reports to Associate Director, Medical Records
- Serves as a liaison between patients, physicians, and other healthcare providers regarding medical record requests
- Processes requests for patient health information in accordance with company and facility policies and procedures
- Standardizes all records that are contained in the medical record in collaboration with medical and administrative staff
- Documents all medical records request by creating telephone encounters in CHN’s Electronic Medical Record system, eClinicalWorks (“eCW”) and assigns to appropriate staff
- Responds to requests for patient records both internally and externally via phone, email, and fax
- Maintains working knowledge of company and facility software
- Maintains a working knowledge of third-party vendor software and processes
- Scans and uploads records to eCW
- Engages in ongoing training, education, and professional activities to enhance knowledge, skills, and professionalism
- Inform manager of work, site difficulties, and/or fluctuating volumes
- Performs other related duties as assigned

- High School Diploma or GED
- Bilingual Spanish is a preferred
