Medical Office Licensed Practical Nurse

About the Role

Join our intimate concierge practice where you’ll truly make a difference in patients’ lives. This role offers the opportunity to practice meaningful, personalized healthcare with a manageable patient load of 7-10 patients per day, allowing you to build genuine relationships and deliver high-touch care. You’ll work closely with a physician in a collaborative partnership, utilizing advanced testing and expanded clinical responsibilities to provide exceptional patient experiences.

Responsibilities

• Build meaningful relationships with a select patient panel.
• Work closely with the physician in a collaborative partnership during patient examinations and procedures.
• Implement and manage Chronic Care Management (CCM), including accurate documentation, patient enrollment, monthly call compliance, and revenue contribution.
• Provide direct patient care including vital signs, medication administration, injections, immunizations, wound care, and diagnostic testing within scope of practice.
• Manage patient records by accurately inputting information into electronic health records (EHR) and maintaining confidentiality in accordance with HIPAA regulations.
• Coordinate comprehensive care plans and follow up with patients personally.
• Serve as a primary point of contact, responding to patient needs with care and attention.
• Apply expanded clinical skills in a thoughtful, patient-centered environment.

Requirements

• Graduate of an accredited Practical Nursing program.
• Current Licensed Practical Nurse (LPN) license in the state of practice.
• Current American Heart Association BLS Certification.
• Phlebotomy experience or demonstrated proficiency in blood collection.
• Minimum two years of clinical nursing experience, preferably in primary care, family medicine, concierge medicine, or an outpatient setting.
• Strong clinical, communication, and patient service skills.

Preferred Qualifications

• Experience in a clinical setting, particularly in outpatient practices.
• Strong clinical assessment and triage skills.
• Familiarity with specific EMR systems (e.g., EPIC, Athena).
• Experience with patient education and health promotion.
