Medical Nutrition Coordinator

About the role

We are hiring for both remote and local employees. This role performs patient follow-up, primarily responsible for evaluating patient adherence to physician orders.

Responsibilities

• Provides patient education as an integral part of patient care, tailored to patient needs and policy
• Performs complete and professional patient assessments in accordance with policies and procedures
• Maintains current knowledge of enteral home care techniques and relevant enteral therapy concepts
• Represents the company through professional personal appearance, patient care activities, and content of written reports
• Gathers necessary information from referral sources to ensure insurance coverage for each enteral patient
• Follows up on necessary paperwork to ensure payment for services rendered
• Maintains patient records in a complete and organized manner in compliance with accreditation standards
• Drives to admit, deliver, and pick up items such as enteral formula, feeding pumps, IV poles, and related feeding supplies to patients' homes or places of business
• Provides patient care, treatment, and service in a continuous and coordinated manner in settings appropriate to meet patient needs
• Promotes safe, effective patient and organizational environments, as well as proper feeding equipment use
• Participates in ongoing education, including in-services, training, and other activities to maintain and improve individual competence
