Medical Director & Skilled Nursing Facility Primary Care Physician (MD/DO)
Position Overview
We are seeking a compassionate and motivated Physician (MD/DO) to join our team in a dual-role position providing on-site primary care services within skilled nursing facilities while serving as Medical Director. This role includes fully funded participation in Certified Medical Director (CMD) training, offering an excellent pathway into clinical leadership. This opportunity is ideal for physicians seeking a flexible and predictable schedule while continuing their existing practice.
Clinical Responsibilities
- Provide on-site primary care services to residents in skilled nursing facilities
- Assess and manage acute and chronic medical conditions
- Evaluate residents experiencing significant changes in condition
- Collaborate with interdisciplinary care teams and facility leadership
- Supervise and support NP/PA clinicians delivering routine and regulatory visits
- Participate in quality improvement and patient care initiatives
After-Hours Coverage
- Participate in a structured and predictable after-hours call schedule
- Manage urgent clinical issues through telehealth coverage
- Telehealth encounters are provider-delivered and billable
- Call volume is well-supported and designed to remain manageable
Medical Director Responsibilities & CMD Training
Physicians in this role will also function as Medical Director for assigned skilled nursing facilities while receiving sponsored Certified Medical Director (CMD) training. Areas of leadership development include: Quality Assurance & Performance Improvement (QAPI), Regulatory compliance and survey readiness, Interdisciplinary team leadership, Skilled nursing facility operations and best practices. CMD certification is not required to begin. All training expenses are fully funded.
Compensation
- Clinical Services: Uncapped RVU-based compensation, supported by stable and predictable visit volume, competitive earnings from both on-site and provider-delivered telehealth services
- Medical Director Role: Separate fixed monthly stipend, not RVU-based, not dependent on facility census, consistently paid regardless of visit volume
Qualifications
- MD or DO degree required
- Board Certified or Board Eligible in one of the following: Internal Medicine, Family Medicine, Geriatrics, Emergency Medicine
- Active Kentucky medical license (or ability to obtain)
- Interest in post-acute care, leadership development, and community-based medicine
