About the role

The Medical Director will provide clinical leadership and oversight of program team members while serving as an active medical provider, delivering comprehensive, high-quality person-centered care across the health care continuum.

Responsibilities

  • Directly manage a multi-disciplinary team of nurse practitioners and physicians to ensure coordinated and high-quality care delivery.
  • Track and analyze clinical performance metrics related to health outcomes and utilization rates; use data to identify areas for improvement and implement strategies to enhance care quality.
  • Participate in community stakeholder meetings and address care-related issues for residents.
  • Provide education and guidance to clinical team members on geriatrics, complex care, and palliative care.
  • Actively participate in provider meetings and quality improvement initiatives to improve patient outcomes.
  • Work cross-functionally with other leaders to design, lead, and implement strategic initiatives improving clinical, financial, and operational performance.
  • Provide primary medical care services to patients in their place of residence.
  • Collaborate with program nurse practitioners and interdisciplinary team members to manage complex patients through in-person and virtual rounds.
  • Participate in a telephonic on-call program to support team members after hours and weekends.
  • Provide immediate support for urgent care needs, including changes in medical condition or goals of care discussions.
  • Maintain accurate and up-to-date electronic medical records (EMRs) and other documentation.
  • Adhere to regulatory requirements, coding/documentation standards, and quality measures set by Medicare and other authorities.
  • Actively support SCAN’s vision and goals.
  • Other duties as assigned.

Requirements

  • Doctoral Degree (M.D.) required.
  • Unrestricted State Medical License required.
  • Geriatrics/Palliative care experience.
  • 5+ years of related experience as a physician, preferably with home-based visits, geriatrics, chronic disease management, and hospice and/or palliative care.
  • Experience with virtual care/telehealth preferred.
  • Knowledge of complex chronic diseases such as dementia, diabetes, heart failure, and COPD.
  • Ability to identify team needs and coordinate with resources/programs appropriately.
  • Knowledge of HCC and ICD-10 coding and documentation.
  • Knowledge of wound care and management.
  • Knowledge of CMS guidelines and Medicare Advantage Managed Care.
  • Comfortable initiating goals of care and end-of-life discussions.
  • Strong interpersonal skills and a change-agent mindset to build relationships with healthcare teams and patients.
  • Cultural competence and sensitivity; multilingual skills preferred.
  • Knowledge of and ability to maintain HIPAA requirements.
  • Adaptable, open to change, and skilled in problem-solving.
  • Basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and openness to new technology.
