Job Responsibilities
- Ensure timely scheduling and adherence to regulations for all medical appointments.
- Prepare clinical consultation forms for all medical appointments.
- Collaborate with RN and Residence Manager to ensure adequate staffing for appointments.
- Accompany individuals/staff on medical appointments as possible.
- Review and sign all medical documentation, then file in individual’s binder.
- Ensure all medical appointments are up-to-date.
- Collaborate with RN to ensure lab work is up-to-date.
- Document and implement physician recommendations.
- Create and maintain a weekly/monthly medical appointment calendar.
- Maintain and obtain AMAP certification.
- Submit prescriptions to ChemRx Pharmacy upon receipt.
- Count/record medications upon delivery; ensure proper counting and recording.
- Check “Client-Specific Sheet” when new medications are delivered.
- Reorder medications when supply is insufficient for at least five days.
- Ensure ChemRx Pharmacy receives all original prescriptions.
- Log and monitor control substances weekly to ensure compliance.
- Discontinue medications when indicated by physician.
- Ensure Medication Room/Cart is clean and orderly, supplies sufficient.
- Order medical supplies as needed.
- Ensure AMAP signatures are on MAR before first administration each month.
- File MAR documentation monthly.
- Prepare off-site medication as per OPWDD regulations and agency policies.
- Incorporate documentation of off-site medication into individual records.
- Assist RN in maintaining and updating individuals’ medical binders.
- Assist RN in completing forms: infection control (monthly), medical appointment (monthly), nursing report, resident report (monthly).
- Complete all requested paperwork by RN, Resident Manager.
- Schedule and complete pharmacy reviews quarterly.
- Monitor and report bowel movement chart issues to RN and Manager.
- Perform additional duties as requested by RN, Residence Manager/Assistant Residence Manager, Program Coordinator, Program Director.
Additional Duties
- Assist with morning routine with individuals: ADL outcomes, making beds, breakfast clean-up, making breakfast as needed and/or requested.
- Assist with dinner preparation if all medical coordinator responsibilities are completed.
- Assist individuals when they return home from program: make lunch for next day, afternoon tooth-brushing, laundry as needed and/or requested.
- Complete laundry if all medical coordinator responsibilities are completed.
- Perform food shopping if needed for dinner and if time is available.
Qualifications
- Comprehensive knowledge of OPWDD regulations.
- Strong organizational and interpersonal skills.
- Good oral and written communication skills.
- Valid NYS Driver’s License.
- Computer literate.
