About the role
A Medical Care Manager contributes to AHF by utilizing their experience, skills, and education together with a thorough understanding of the Ryan White Care Act Program within their specific area. With a caring and knowledgeable focus, you will work with our providers and healthcare center team so that every Ryan White client receives the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood.
Responsibilities
- Work with our providers and healthcare center team to ensure every Ryan White client receives the support they require and access to available resources.
- Meet with clients and complete Needs Assessments, develop Action/Care Plans, and provide ongoing education to patients so that they understand their diagnoses and their medications.
- Understand mental health needs and basic needs such as housing, food, and clothing and develop relationships with programs and agencies serving those needs and facilitate referrals, support, and assistance for your patients who lack those services.
- Enhance the reputation of AHF by utilizing your case management experience, identifying strengths and opportunities for growth, and assisting clients with realistic goal setting.
- Bring the essence of the Ryan White Care Act - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population by establishing Needs Assessments, Action/Care Plans, referrals, and support.
- Contribute to AHF’s success in meeting the deliverables of our Ryan White contract and measures established by the HIV/AIDS Bureau (HAB) and the Department of Health and Human Services.
- Be a passionate advocate for our top initiatives while ensuring the AHF Core Values and Mission stay top of mind with all that you do.
- Work as a part of a close-knit team to be the liaison between provider and client, facilitating the shift from leaving the exam room to implementing the optimum plan of care and putting it into action.
- Provide brief interventions focused on a long-term achievable goal, working together with clients each step of the way towards viral suppression, better health outcomes, and quality of life.
Requirements
- BS/BA in Social Work, or other social service field with two (2) years of direct service delivery.
- Familiarity with housing case management.
- Familiarity with housing entitlement systems and procedures.
- HOPWA Training Modules.
