Medical Assistant Care Management

About the role

This team member will be part of our centralized Ambulatory Care Management department alongside Care Coordinators/Care Managers, MSWs and a RD. They will contact patients after emergency room visits to provide transitions of care support as well as manage incoming patient calls from patients after their hospital discharge. The team member will manage referrals to our care management team (RNs, RD, MSWs) and provide support for scheduling, rescheduling, cancelations and no shows. The team member will complete benefit verifications for our statewide team for recently discharged patients to support care management workflows.

Requirements

• High School Diploma or equivalent
• Successful completion of a competency-based medical assistant training program
• 1 year of relevant experience in a physician practice or healthcare setting
• AHA or ARC Basic Life Support (BLS) Certification Required within 90 Days
• AAMA Certified Medical Assistant (CMA), AMT Registered Medical Assistant (RMA), NHA Certified Medical Assistant (CCMA), NCCT Certified Medical Assistant (NCMA), NAHP Certified Medical Assistant (NRCMA) Required within 120 Days

Qualifications

• Ability to manage multiple tasks and prioritize effectively
• Knowledge of medical terminology and procedures

Pay

$3,000 sign-on bonus for external applicants or current CHW MA Apprentice Students only

Schedule

The first six weeks would be a combination of work from home and in office time at either 60th St. or Corewell Health Place on Monroe. Weekly Scheduled Hours: 40 Hours of Work: 8 a.m. to 4:30 p.m. Days Worked: Variable Weekend Frequency: N/A
