About the role

You’ll partner directly with leading cardiology practices to manage patients with stable cardiovascular conditions—conducting virtual assessments, coordinating follow-ups, monitoring progress, and educating patients on lifestyle changes. You’ll collaborate with providers working at the care site to ensure continuity and quality, all while working with streamlined workflows and purpose-built tech platforms.
