Medical Assistant
About the role
The Population Health Medical Assistant supports primary care teams within the network, either remotely or from a primary care office, to enhance compliance with population health initiatives such as annual wellness visits, quality care gap closure, and in-network referrals. The role does not involve direct patient care but involves executing standing orders for patients, scheduling appointments, and closing care gaps.
Responsibilities
- Regularly review patient charts for screening needs, quality gap closure, and care coordination requirements.
- Utilize standing orders developed by primary care physicians to place orders for screening and quality gap closure needs, working under the appropriate physician licensure.
- Manage in-network referrals for all ACN value-based lives.
- Conduct member outreach to schedule patient visits for campaigns such as annual wellness visits, well-child visits, breast cancer screening, and colorectal cancer screening.
- Collaborate with primary care teams and ACN clinical performance consultants to effectively manage ACN value-based lives.
- Transfer patient data regarding quality care gaps from clinic medical records to ACN population health management platforms.
- Proactively work payer care gap and frequent flyer/high-risk reports for all ACN value-based lives.
- Understand ALOS, readmission rates, and Gaps in Care.
- Perform other duties as assigned by a supervisor.
Essential Skills
- Proficiency with electronic health records and appointment scheduling.
- Experience with EMR systems.
- Experience traveling to different physician offices in the network to address patient population care gaps.
- Experience in reviewing standing orders for patients and conducting patient outreach to promote wellness visits and other screening appointments.
Additional Skills & Qualifications
- At least three years of experience as a Medical Office Referral Coordinator, Medical Assistant, or Health Plan/Managed Care Organization Utilization Technician.
- Diploma from a Medical Assistant program.
- Familiarity with CPT codes, ideally CPT-2 codes.
- Experience and comfort with Microsoft Office Suite (Outlook & Excel).
- Experience with quality measures like care gap closures, pre-visit planning, HEDIS measures, and patient outreach.
- Strong ability to follow instructions, analyze data, coordinate tasks, and manage workload effectively to meet business goals.
- Excellent verbal communication skills, especially for leading office staff and providing feedback.
Work Environment
This role is based out of a central hub but requires floating to different provider offices across Maricopa County. The work schedule includes working from the ACN office in Arcadia on Mondays, from clinics in the Central Phoenix Maricopa County territory from Tuesday to Thursday, and potentially working from a clinic or from home on Fridays depending on clinic hours. Hours will be dependent on the clinic hours but generally are an 8 hour shift from 6a-7p M-F.

This is a Contract to Hire position based out of Phoenix, AZ.
Pay And Benefits
The pay range for this position is $22.00 - $26.00/hr.
