Responsibilities

  • Collaborates with Service Coordinators to support individuals, families and service providers who are connected to the Individual and Family Support Department in relation to Medicaid, healthcare and public assistance benefits such as: applications, re-determinations, denials and generalized questions specific to the state of NH.
  • Maintains a lapsed Medicaid report that is provided the current status of individuals without Medicaid or waiver coverage. Distributes report to management.
  • Serves as back-up to the Funding Specialist for Fiscal agenda preparation, distribution and Renewal budget information.
  • Provides support to staff in the Individual and Family Support Department and all other departments through consultation as necessary in relation to Medicaid redeterminations and benefits.
  • Maintains 3-year spreadsheet of upcoming Individuals to receive services and their Medicaid eligibility status.
  • Works with NH CarePath staff to navigate requirements for Individuals to obtain Medicaid prior to the start of services.
  • Affords support to keep Medicaid active for billing.
  • Works as the direct liaison between the Individual and Family Support Department and DHHS in relation to all Medicaid applications, denials, questions and concerns that become escalated.
  • Creates and provides an ongoing training series around Medicaid and benefits through a variety of tools such as in-person trainings for staff, workshops for individuals/families and webinars etc.
  • Meets with new Service Coordinators to provide introductory education regarding Medicaid and public assistance benefits.
  • Participates in ongoing NH CarePath meetings quarterly and reports out pertinent information to the Individual and Family Support Department after each meeting.
  • Participates in weekly DAADS meeting with BDS assigned Liaison.
  • Services as point person for CMS PERM audits.
  • Participates in Community Bridges outreach efforts, such as local Transitional Fairs.
  • Utilizes a person-centered approach when assisting families/individuals to make the necessary connections to the Department of Health and Human Services.
  • Will be on Intake committee for resource on Medicaid and eligibility needs/questions as they relate to intake and complete process of supports for clients in agency.
  • Develops and maintains contact with appropriate professionals in entitlements agencies.
  • Other duties and projects as assigned.

Qualifications

  • Education: Associate's Degree in Human Services or related field required; Bachelor's Degree preferred. Equivalent 2 to 4 years of Medicaid/eligibility experience may be substituted for degree.
  • Experience: Previous social service and Medicaid experience required. Must know and have current knowledge and understanding of service coordination processes for funding and budgets as they relate to services with DHHS. Must be able to work collaboratively with service coordinators and finance on process and procedures as they relate to Medicaid funding.
  • Basic understanding of Medicare, and Managed Care Organizations of NH is recommended, but not required.

Compensation And Benefits

  • There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role.
