Role Overview The Wellness Nurse LPN is responsible for monitoring resident health, supporting medication management, and maintaining clinical quality within a senior living community. Key Responsibilities Conduct assessments such as move-in, semi-annual, and change-of-conditionPerform monthly wellness visits and monitor health trendsFollow up on falls, incidents, and hospital returnsTranscribe physician orders and ensure accurate documentation of MAR/TARComplete medication audits and oversee medication complianceCollaborate with families, physicians, and care staffSupport infection control and regulatory standardsEducate staff on documentation and medication practices Qualifications & Skills Active LPN license in good standingGraduate of an approved nursing programAt least 1 year of experience in senior care or clinical settingsStrong assessment, documentation, and communication skills The role emphasizes proactive health assessments, early intervention, and clinical oversight to promote resident wellbeing.
