About the role

• Provides quality care and assistance with activities of daily living for residents.
• Assists with the administration of prescribed medications within the established scope of practice.
• Maintains working knowledge of all policies and procedures pertaining to the position.
• Reports resident incidents, care concerns, or issues to appropriate individuals (e.g., HCD, RN/LPN, or ED).
• Performs housekeeping duties as directed, such as making resident beds, cleaning apartments, common areas, and dining room.
• Responds promptly to resident calls for assistance or emergency alerts.
• Assists in planning and conducting activity programs for residents.
• Follows confidentiality policies regarding release of resident information.
• Assists residents to and from the dining room and activities.
• Checks on residents to ensure attendance at meals and activities; monitors ill residents and reports status or concerns.
• Communicates verbally and in writing to report resident status, upcoming community information, appointments, admissions, and transfers.
• Assists new residents with move‑in transition, providing emotional support.
• Attends all scheduled medication trainings.
• Completes medication cart audits as assigned and maintains the medication cart as directed.
• Passes all medication administration observations.
• Communicates medication resident needs to the HCD and documents in the Daily Log.
• Re‑orders medications, faxes to pharmacy, and communicates with resident family or responsible party as needed.
