Responsibilities

• Makes assessments and creates personalized service plans for residents
• Ensures proper follow-through and documentation for residents with changing clinical care needs
• Serves as a liaison between resident support services and families
• Maintains clinical quality assurance
• Supports the Wellness Nurse with completing monthly wellness visits and PSP
• Notifies the Wellness Director and/or Executive Director of any changes in medication or service levels
• Absorbs and documents resident vital signs monthly
• Provides resident care as needed

Requirements

• Certified Medication Aide and/or meets state requirements for medication aide
• At least one year of experience working with elders or disabled individuals, preferred
• Demonstrated leadership skills
• Experience in assisted living industry and dementia care or skilled nursing, preferred
• Willingness to work weekends, evenings, and flexible hours
