Med History Pharmacy Intern
About the role
The Pharmacy Medication History Intern at Regions Hospital is responsible for obtaining and documenting medication histories for patients admitted to the hospital. This includes interviewing patients or their families, contacting retail pharmacies or primary clinics, and using medication history software queries. The intern then enters this information into the electronic medical record and communicates its completion to pharmacists.
Responsibilities
- Obtain and document medication histories from patients or their families
- Contact retail pharmacies or primary clinics to gather necessary information
- Query medication history software to supplement information
- Enter medication history information into the electronic medical record
- Communicate completion of medication history to pharmacists
- Perform other projects as assigned, but do not perform duties legally restricted to a registered pharmacist
Requirements
Interns must have general knowledge of pharmacy principles and practices, knowledge of drug classifications, medical terminology, and medical abbreviations. They should have working knowledge of common outpatient medication doses and forms. Proficiency in reading, writing, and speaking English is essential, along with strong interpersonal and communication skills. The ability to prioritize work and work independently is also important.
Qualifications
- Currently enrolled in an accredited pharmacy program or a graduate of an accredited pharmacy program
- Licensed as a pharmacy intern with Minnesota State Board of Pharmacy before starting
- Preferred qualifications include being a pharmacy student finishing first or second professional year with the ability to work a minimum of every other weekend rotating schedule plus one week night per week
Skills
Knowledge of computer systems, automated systems, and medical record software is preferred. Understanding of hospital or retail pharmacy procedures for filling, dispensing, and billing prescriptions is beneficial. The ability to analyze problems, troubleshoot, and implement fixes is also important.
