About the role

The MDS Nurse and RN at Good Samaritan – Woodland are dedicated to providing quality care and service in a Christian environment. This position works in a 42-bed long-term skilled care and transitional care facility in Brainerd, MN.

Responsibilities

  • Evaluates care provided to each resident and keeps care plans current.
  • Assists in assessment and evaluation of potential admissions.
  • Completes resident assessments upon admission, quarterly, and annually.
  • Collaborates with residents, families, advocates, and interdisciplinary colleagues to ensure ongoing care.
  • Utilizes the nursing process to provide individualized nursing care to patients.
  • Coordinates care, assesses residents, educates residents, and implements various nursing interventions.
  • Serves as charge nurse and delegates tasks to licensed and unlicensed assistive personnel.
  • Ensures safe and reliable administration of resident care and resolves care and service concerns.

Requirements

  • Graduate from a NLN or AACN accredited nursing program.
  • Bachelor’s Degree in nursing preferred.
  • One year experience working as a licensed nurse with the senior population preferred.
  • Specific services/positions may have additional education, training, or experience requirements.
  • Resident Assessment Coordinator-Certified (RAC-CT), management or supervisory experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing.
  • Maintains required certifications and completes CEUs.

Qualifications

  • Preferred graduate from NLN or AACN accredited nursing program.
  • Bachelor’s Degree in nursing preferred.
  • Minimum of one-year experience working as a licensed nurse with the senior population preferred.
  • Specific services/positions may have additional education, training, or experience requirements.
  • Resident Assessment Coordinator-Certified (RAC-CT), management or supervisory experience preferred.
  • Currently holds an unencumbered RN license with the State Board of Nursing.

Skills

  • Independent judgment in planning, organizing, directing, and evaluating activities of nursing staff.
  • Collaboration with interdisciplinary teams.
  • Use of the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation).
  • Ability to delegate tasks effectively.
  • Knowledge of age and population-specific standards.
