Responsibilities

• Complete the RAI process and care management process accurately and timely from admission to discharge, adhering to company policies, procedures, and Federal, State, and Certification guidelines.
• Handle Minimum Data Set, discharge and admission tracking, and other related tasks.
• Coordinate information systems operations and education for the clinical department, with direction from the VP of Clinical Reimbursement.

Requirements

• Current, unencumbered, active license to practice as an LPN/LVN or RN in this state.
• Three (3) years of MDS experience in a nursing care facility preferred.
• Training in rehabilitative and restorative nursing practices.
• Working knowledge of managed care rules, regulations, and guidelines.
• Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary for providing quality care.
